Our school district offers a full range of educational services for families ranging from pre-school through twelfth grade.  All students k-12 are registered in a central registration office located at 425 Yoctangee Parkway, in the Trent building, which is located in front of the high gymnasium.  Once students are registered, their information is processed and they are scheduled to begin attending their new schools.   This “one-stop shop” makes completing paperwork and registration requirements easier for families. 

We will need the following information when enrolling:

• Birth Certificate

• Social Security Card

• 2 Proofs of Residence

• Photo ID

• Immunization Records

• Custody paperwork if applicable

• Open enrolled students, complete this form

We are happy to announce that we will be using FinalForms now for back-to-school and new student forms.  Please log in to Chillicothe Final Forms to begin the process.  All back-to-school "paperwork" will be done through this program.  You can also click on parents on our website and see the link for final forms.  Instructions for Final Forms are here.
